3rd Street Youth Center & Clinic and the Department of Homelessness and Supportive Housing are partners in the City’s newest initiative for youth experiencing homelessness—the Lower Polk TAY Navigation Center.
The Lower Polk TAY Navigation Center operates 24-hours a day, seven days a week, and is free to young adults, ages 18 to 27, who have been referred from San Francisco’s Coordinated Entry System. Located at 700 Hyde Street on the corner of Post and Hyde, the Navigation Center is centrally located to San Francisco neighborhoods like Hayes Valley, the Mission, and Downtown and is a short walk to BART and MUNI.
We Offer
Housing: Youth will have a safe, clean, and comfortable place to stay, and a secure locker for their personal items. We welcome your service animals, companion animals, and personal pets.
Meals: Youth will enjoy three meals every day, plus snacks.
Personalized Support: We will work with youth one-on-one to identify and achieve their goals. One-on-one support includes free counseling and case management.
Health & Wellness Services: We will help youth improve their physical and mental wellbeing.
Education: We will help youth continue with high school or college.
Employment: We will help youth get ready for work and find a job.
